My test results say " Large Hiatal Hernia and Mod erosive gastropathy" should I be expecting a surgery? Also can those be the cause of almost-constant headaches?

The presence of a hiatal hernia by itself is not a reason for you to have surgery. if it is associated with reflux symptoms that are severe or not responding to medical therapy, then surgery may be needed.
The moderate erosive gastropathy is common and treated medically.
It is unlikely that the results above are linked to your headaches.
